Dr. Anthony Ortegon is a designated civil surgeon of USCIS.

Client must obtain the most current I-693 from Immigration Direct to bring to the appointment for he medical exam. The cost of the exam is $110 per person.

Client must bring an interpreter or have an interpreter fill out and sign I-693 if they are not fluent in English.

Clients over the age of 2 are required to receive a PPD to check for Tuberculosis and come back to the office in two (2) days to have it read. If there is a significant reaction measured to be over 5mm, a chest x-ray is required at additional cost.

Clients 15 years and older are required to have a RPR blood test to check for Syphilis and also a urine test to check for Gonorrhea. The cost of these test is not included in the $110 exam fee and can be paid to the lab at the time of service or can be billed separately.

All clients must bring an up to date vaccination record to their medical appointment. All adults must have an up to date TdaP (tetanus and diphtheria) as well as current MMR (measles, mumps rubella). If client has not had chicken pox as a child, they must receive the Varicella vaccine. If the client has had the disease it will be noted on the vaccination form. During season flu season, clients must also have received the flu shot. If client is over age 65 they must have received the pneumonia vaccine. All clients under the age of 18 must have up to date vaccine record for their age.
